- Insurance cards so we can process claims timely and correctly
- Co-payment
- Prescription eyeglasses / sunglasses
- Wear current contact lenses to your appointment
- Contact lens prescriptions and / or the boxes
- Complete list of medicines, including over the counter medicines
- Any eye drops you may be using
- Family history of eye diseases
- Name of your primary care physician
